Bates was born in Memphis, Tennessee, as the youngest of three daughters. Her father, Langdon Doyle Bates was a mechanical engineer, while her mother Bertye was a homemaker. Bates was dedicated to her education, and she graduated early from White Station High School in 1965. She joined Southern Methodist University, where she graduated in 1969 with a theater major.

In 1970, Bates relocated to New York City to focus on acting. She did several odd jobs in New York City including as a cashier at the Museum of Modern Art because she struggled to find work as an actress.

In 1970, the actress got a minor role in the comedy Taking Off. She continued to struggle to find more roles after the first minor role. During an interview with the New York Times, the actress revealed that more than one casting agent told her she was not sufficiently attractive to be an actress.

Despite struggling to get work, she started getting more roles towards the end of the 70s and early 80s. The New York Times wrote that Bates was one of America’s finest stage actresses of the 80s.

Bates found success as an actress in 1990 after she starred in the horror film, Misery. The film was a huge success and her performance of Annie Wilkes was applauded. Soon after that, the actress began starring in numerous roles, as well as working behind the screen as a director. Some of the works she directed include NYPD Blue, Great Performances, and Homicide: Life on the Street.
